The Special Adolescent Clinic was established in 1991 to CONNECT 2 PROTECT and deliver primary health care to adolescents 13-24 years of age infected with HIV. This program is staffed by professionals experienced in adolescent medicine including a physician, ARNP, peer educator, case manager, researchers, linkage specialist and psychologist.
The Adolescent Linkage Specialist will meet the new client the same day he/she is referred to the SAC clinic. During this initial meeting, the Linkage Specialist will build a rapport by briefing the new client of our pediatric HIV primary care clinic, explain services provided, and exchange contact information. The LTC Specialist will also introduce the new client to the Adolescent Medicine team (MD, ARNP, Peer Navigator, Case Worker, Psychologist) and set up their first appointment with our practitioners. Bus passes and/or LYFT services will be provided if the new client is without transportation.
